LWS offers medially necessary home health care services specifically tailored for individuals, from birth to second childhood. Services include activities and support unique to each family’s circumstances. Services may be temporary or long term depending on an individual’s medical or mental status and/or rehabilitation or recuperation. Our specialty trained staff will ensure that you, or your loves ones, are able to safely live and independently as possible in the comfort of familiar surroundings. A paraprofessional is supervised by a dedicated case coordinator and a registered nurse. A registered nurse evaluates needs, ensures the scope of service is being met under the individual’s physician care plan and that the healing process is as comforting as possible.

Based on one’s physical, mental and social needs, a certified paraprofessional and or professional will assist with activities of daily living such as:
